Health Care - Chronic Health Planning

Kia ora koutou, My name is Tayla & I have moved to a self employed role as of 2024, I hoped it would allow me some flexibility around life & return to study to pursue my Masters (however, self employed life is harder than I thought ).I am currently contracted to Taikura Trust who provide support and funding for people in the Auckland region who have diagnosis' such as long term Chronic Health, Neurodiversity & Disability from ages 0-65. These include children with developmental delay/Learning difficulties from adults with neurological conditions. My role is to assess these peoples needs & complete care planning by making community referrals to range of health professionals and calculate these individualised Funding Packages so that our clients can access a range of different community services.
